When you order an instrument on this website, you will automatically be sent a scoring manual. Please follow the scoring instructions in the manual.

Permission to use any questionnaire from this website is obtained by completing the form on the download page, providing all necessary information including intended use of the questionnaire.

This happens if you do not make a selection under both the "Language" and "Questionnaire" columns. Please be sure to check at least one choice in each column.

Since our questionnaires are sent as email attachments, your email system may block them, believing them to be spam or viruses. Please check your junk mail folders and filters. If you do not receive the questionnaires within 24 hours, please contact us.
