This website provides current information about validated questionnaires concerning overactive bladder (OAB), benign prostatic hyperplasia (BPH) (coming soon), and sexual health. It is designed to be a resource for up-to-date versions and translations of many available instruments used to assess patient reported outcomes. Please note that the instruments are designed to be used as screeners and for clinical research but not diagnostic tools.
The questionnaires found under each of these main topics may be ordered free of charge by going to the order page. You will note that translations are available in a variety of languages. All questionnaires will be emailed with a scoring manual.
